Case Management

Diversified Rehabilitation Group recognizes the human and business benefits of efficient and well-planned medical/psychological and vocational rehabilitation interventions.

Our Case Management goal is to assess, coordinate, monitor and evaluate physical/psychological rehabilitation in conjunction with the identification of suitable work (including transitional work duties when required). Diversified Rehabilitation Group assists, coordinates and monitors an individual's Case Management needs with the Insurer, Medical/Psychological Communities and Employer/Union Groups. The fundamental steps in the Case Management process are illustrated below.

Assess Return to Work Capability

The Case Management of claims is the ability to accurately assess an employee's Return to Work potential. Information pertinent to understanding of employee’s ability to return to work may include: accurate medical information; critical job demands analysis; an understanding of the dynamics of the work environment; and an evaluation of the employee's ability to participate in an appropriate rehabilitation plan.

Rehabilitation Planning

The Rehabilitation Plan is developed in conjunction with medical professionals and disabled workers. Diversified Rehab communicates with service providers and initiates, implements and coordinates the appropriate rehabilitation interventions.

Treatment Plan and Transitional Work

Our qualified professional evaluates the employee's rehabilitation progress relative to the Return to Work goal. Should the Rehabilitation Plan be found to be inadequate, a revised plan with further modifications of rehabilitation services is developed and implemented. Diversified Rehab. will also identify any required job modifications special equipment/aids and help arrange these when applicable.
